I was diagnosed with IBS-D 3 months back and my condition was referred as Post Dysmotality Syndrome. Whatever I eat at night I kind experience an aggressive bowel movement 1st thing in the morning (Not Diarrhea) sometimes stool in less quantity and I experience bowel movement atleast twice between 6 am to 9 am...In between I have no stomach pain or excessive gas...But alcohol seems to trigger next morning. My blood tests are normal, Weight has reduced like 6 kg in past 6 months, ultrasound is normal as well...If I press my stomach below belly button I feel pain like inflammation otherwise there isnt any pain...I have lost bit of weight and my doctor says there isnt anything to worry clinically....I need your advise related to all the reports attached and what are your observations?
Also I feel a strange sensation (sometimes only or mostly at night after eating dinner and lying on bed)..Its like bubbling inside with a sensation towards anus...Like a fart happening inside or like a vibration bubbling....Your views on that one as well please?

Have seen your details and i appreciate your concerns.

Actually IBS is a disorder of exclusion which means that even when after thoroughly investigating the patient nothing turns up to be wrong patient is labeled as having IBS. Basically in IBS there is no pathological problem with your gut but there is problem with overall functions of gut.Basic problem in treatment is that always almost every patient presents with different set of symptoms and its not necessary that all symptoms are there in every patient.
As seen in prescription you were given rifagut and colispa. Right now as you are symptomatic i would like to start you on a course of rifaximin for 14 days 400mg once with a anti spasmodic anti inflammatory like CIBIS thrice daily for 5 days.
However basically you have to understand that IBS can't be treated by medications as it is a functional disorder and life style modification plus food habit changes along with medications would help you in long runs.
Some suggestions are-
Take a tablet of Nexium 40 mg once daily in morning this would help you with gas along with it take any probiotic like flora zn twice daily.They are good bacterial culture and would help you in overall digestion of food.
If you feel constipated then take any laxative like lactulose.
Eat simple light vegetarian food.
Take 200ml of curd twice daily.
Avoid heavy and oily foods.
Try to figure out what suits you and what not.

Hello again
Basically rifagut is not absorbed into blood,It acts locally in intestines hence is a very safe drug.
Rifagut is an antibiotic(new generation) and would cover the harmful bacterias.
As said earlier there is no cure but yes symptoms could be controlled fully and you could live a normal life
What are you anxiety levels on a scale of 1 to 10?
And what about your sleep?.
Initially take Nexium,Rifagut etc for 10 days and tell me how you feel.
After that depending on response we can proceed with treatment.

Very likely it can be stress related.
Stress stimulates the nervous system which effects gut also.
Start taking herbal supplement Lactium and ask you father to enclose 20-30 tablets of clonazepam 0.5mg along with other medicines.
Its a anti anxiety medication and lets see if you respond to it.

1) I pass large amount of gas as soon as I wake up in the morning
2) I take a warm glass of water and in 5 minutes I am in toilet
3) Sometimes I have to exert and sometimes the BM is effortless
4) The urge to have a big breakfast meal has reduced from past 10-15 days, though I still go for a Glass of milk (Soy Milk) + Scoop of 100% Whey Protein and then 2 bananas in the office
5) In between my transit time to office I feel the pressure again and usually go another time in office
6) The motion in the morning is soft and sausage shaped (Sometimes floating, sometimes submerged completely)
7) In most of the instances I see food particles on it (Carrot, Red Capsicum, corns or peas)
8) After eating dinner I got for 15 min brisk wallk chewing Fennel seeds + Carrom seeds
8) The stool is very rarely watery where it comes with a splash of brown water & solid poop both at the same time (Though this instance is very rare)
9) Yesterday I had 4 chapatis and within 15 min I was passing gas during intervals and continued till I went off to sleep
10) My BM is mostly soft, sometimes pencil thin (Very rare) but have seen mucus as well sometimes (Not always)
11) From past 10-15 days I have a feeling of something stuck in my throat and bit nasuea feeling in the morning (Never vomited though)
12) The Stool shape in morning is lumpy and 2nd BM is medium sized small shaped but soft
13) I sometimes feel fatigue usually during the mid day followed by headache / dizziness which gets better by rest (Though this is not a continuous pattern)
13) My weight loss from Nov'17 till date is related to all the above symptoms? Or you think the food / nutrients aren't getting absorbed properly?
14) I did reduce the intake of Carbs & sweets to a great extent
I do not have any other issues like stomach cramping, or midnight pain in stomach, vomiting
My CBC, Abdomen Ultrasound was done in Dec'17 (With all normal reports, also attached copies for your reference)
The FOBT (Colon Cancer Kit test) was done 2 months back with Negative results
With all the above symptoms and description what is your diagnosis? Is this normal or IBS or Underlying condition or my metabolism getting better or something that needs to be fixed or will remain like this lifelong?

Hello again basically Zantac has different mechanism of action compared to Proton pump inhibitor like esomeprazole.
On one hand while rantac just reduces the excessive secretion of acid by stomach parietal cells ppi's reduces the baseline plus excessive secretion of acid.Hence they are far more effective.
You can take any other over the counter PPI like prilosec twice daily(omeprazole and pantoprazole are over the counter).
Mylanta is ok
Both of them initially for 21 days.
Report me how you feel after first 10 days.Doses and tapering depend on the response of body to symptoms.
Don't forget to add a stress buster as suggested by me.

what does you primary doctor suggested you?I mean i hve seen his prescription but apart from that what he said?

Also, what is the cause of fatigue that happens during the mid day sometimes its really severe and causes dizziness and headache...Is that all part of IBS?

Also with my CBC being normal in Dec'17 do you think I need to repeat them again? Could there be any change in counts?

Answers are-
1)Nexium 20 Mg from the pharmacist and to start with I have taken 2 tablets as per your recommendation (40mg dose). I have also bought Double Strength Probiotic 64 Billion per capsule (Please let me know if thats ok?)-Absolutely ok.
Start having them if possible take nexium empty stomach and probiotic with a cup of yoghurt.
If food is not digested properly or you are having more liquid in stools or mucuos then body would loose more sodium potassium and other minerals with every stool secondly absorption and assimilation of food is not good then obviously during hours of more stress you would feel fatigued or may be dizzy etc.
No test suggested right now.
Some food plans are-
-Whole grains are very beneficial to consume.These can be in the form of XXXXXXX whole wheat bread or cereal, brown rice, whole grain pasta,grains such as whole oats, wild rice, rye, oatmeal and corn are also beneficial and can be taken if suits your fooding pattern.
-Fruits and vegetables should be a significant part of any diet to help in recovering. They are full of essential nutrients and are easy to digest. follow a high carbohydrate diet,Fat contents of diet should be low.However Olive oil, canola oil and flaxseed oil are all healthy fats that are recommended as part of a diet.Try to eat clean 80% of times and satisfy your buds twice a week.
-Healthy and tasty proteins in the form of low-fat milk and dairy products along with lean meats, beans, eggs and soy products can also be a part of a healthy diet.Consume slowly(amount you feel you can digest).
