Question: Hi, My mom had a blister that broke on inside of middle toe. It left a white circle. I have been soaking it and applying Vaseline. The white area has shrunk but not came off. Today it has red around it and her toes where more red. She has been on an antibiotic. I’m not sure what the white is? And what can I do to help this heal? Mom is diabetic. The white area is painful to touch it. I would like to send you a close up picture. Mom is diabetic so things heals slower on her. But her a1c is good and she takes pills with each meal. Please can you help me

It is a corn. It can be removed after the infection resolves. She can continue antibiotics for the time being. In addition she can either do an antiseptic dressing or use a topical antibiotic ointment e.g either neosporin ointment or mupirocin ointment for 7- 10 days. Once pain and swelling and redness subsides it can be either removed or she can use topical keratolytics like Salactin paint. It is a prescription medication and has to be used under supervision and guidance of a local dermatologist.

Hi how would the white area be removed? I’m assuming it is the corn? It is very smooth but the top layer I assume is dead skin. Does that top layer need to be cut? I’m not sure what an anticeptic dressing is? Do I go ahead and soak it daily? Do I add anything to the water besides Epsom salts and dish soap? Should I not rub much on that area? Thank you very much

Hi.
The white area is actually thickened skin which has become macerated due to occlusion. A topical antibiotic ointment or an antiseptic dressing would reduce any secondary infection (along with oral antibiotics) as well as reduce pain and swelling. She may use topical keratolytics (salicylic acid and lactic acid preparation) like watnil lotion or salactin paint after 7-10 days of oral and topical antibiotic. It would soften on keratolytics and it might just come off. Another option is that it can be removed with a cautery machine by a dermatologist.
