Question : Hi recent acute hepatitis late XXXXXXX 18 unknown origin ALT 1800 AST 1450 BIL 220. Still a waitin liver biopsy result. Female age 56. All LFTs coming down last week ALT 142 this week 172 was prescribed prednisalone now down to 5mg from 60 mgs what are your thoughts please

Very likely you are now in recovery phase of hepatitis and enzymes would take about 4 more weeks to come back to normal however a liver rejenuvator like sylimarin can be added daily to help the liver cells recover fast.Its a very potent liver rejenuvator and is available over the counter.
Follow a high complex carbohydrate and low fat diet for next few weeks.
A diet plan is-
-Whole grains are very beneficial to consume.These can be in the form of whole wheat bread or cereal, brown rice, whole grain pasta,grains such as whole oats, wild rice, rye, oatmeal and corn are also beneficial and can be taken if suits your food pattern.

Stop prednisolone in a week or two.
Follow up with results of biopsy.

Thank you for your help, I am a nurse don’t smoke or drink am concerned this could be autoimmune disease nothing else like this in body . All other causes of hepatitis have been ruled out as zi day still waiting biopsy results .I had wondered if I should go back to 10mgs if prednisalone, but to be honest I really don’t want to do this except if absolutely necessary. Or could part of the issue be the steroid ? When my levels peaked I spent a week in surgical ward before going to medicine during that time ALT fell from 1800 to 950 with no treatment but gastrologist decided to use steroids anyway. Hope this is helpful I am a bit sore around liver area large dose of prednisalone had took that away. Please get back to me Many thanks XXXXXXX

Nice to know that you are a nurse and must be aware of side effects of any steroid.Therefore i would suggest you to take the minimal possible dose of it till results of biopsy are there.
However if it is auto immune then steroids and
Immunosuppressants have to be used life long.
As you have responded to steroid chances of the disease being auto immune is unfortunately a bit high however till results of biopsy are there lets keep our fingers crossed.
Don't panic too much now a days patients can live a perfectly normal life on immuno suppresants etc.

Thank you ward sister over 20 years was suddenly very unwell in XXXXXXX however as LFTs had come down I was surprised at even being commenced on this medication. My followip is not until late Nov however I’m sure once result is in they will contact me. My husband is concerned thinks I should just get off theses and see what happens he also is a district nurse. It’s s concern I’m still taking 5 mg and I ll see what next blood are like being done weekly, do sorry that steroids are hurting me. Thank you for your time and patience
