Question : Hey doctor for the last year now I have been having issues with my stomach. Mainly symptoms consisting in the center of my epigastric region including a sense of fullness after eating a small amount, my stomach feeling distended, nausea, belching, gas and a burning/itchy feeling in my stomach along with occasional reflux. In July last year I had a gastroscopy procedure which came up clear and this year the symptoms had really calmed down compared to last year but 3 weeks ago I went to the United States and I am from Australia and in the last week of my stay in the US I had a small case of gastroenteritis with painful diarrhea symptoms which I had in the last week of my 1 months stay there. Ever since I got back home my stomach has been having those symptoms I mentioned above but mainly in my central epigastric region and I have had them for about 2 weeks now.

My doctor gave me this product to try see if it will calm down as I have tried PPI's and other acid suppressors previously last year and they just seemed to make everything worse for me

How long do you think that product would take to work it's mainly natural supplement that's meant to calm the stomach down and would there be any side effects in the short term and long term of taking this supplement based on the ingredients in there and would they interact with any other medication? In terms of my diet what should I be eating to make it better and what should I avoid?

That's more detail link of the medication I am taking but I take it once in the morning 30 minutes before breakfast and once at night 30 minutes before bed and I have been taking it for 6 days now and I haven't noticed a huge difference but I am unsure how long it would take my doctor thinks I have gastritis that was triggered after going to the US? I have also attached my Gastroscopy from last year.

The only side effect I’ve found so far is that I have been kind of gassy taking the medication along with feeling full in my stomach after taking it which goes away with 5 minutes or so.

Firstly answer to your query is natural supplements results depend on the trials done by company selling them. They may take months to show initial effect or may either be totally ineffective on a person. Best time to use them are when you are almost symptom free.
Now lets move to my diagnosis and suggestions-
Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
*Have a wholesome diet rich in fiber. Good fiber sources are vegetables, legumes, fruits and multi grain bread like XXXXXXX
Eat a well- balanced diet with plenty of fiber. Good sources of fiber are fruits, vegetables, legumes, and whole-grain bread and cereal (especially bran).
Drink 1 1/2 to 2 quarts of water and other fluids a day . Fiber and water work together to keep you regular.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Stress reduction has a very important role to play. Yoga and meditation should help.
*Keep taking natural supplement.

Thanks for the very detailed reply it definitely helps clear things up a couple of other things I wanted to ask since you are very knowledgeable.

My doctor also asked me to take this fiber would that help with what you mentioned with increasing fiber intake?:

Along with having this and bone broth:

I also did the IgA and IgG food intolerance test and it came up with me being intolerant strongly to dairy and weakly to eggs, legumes and wheat so should I avoid all of those?

My doctor also wants me to do a heart test since I also experience shortness of breath along with my digestive symptoms but I just have a couple of concerns with it. I have attached an image of the exact type of tests they want me to do but it’s an “exercise echo” heart test. I was just wondering if there is any risks involved in this test with radiation etc.? I just don’t want to be exposed to unnecessary scanning even ultrasounds as I feel it’s not good in the long run and I am still quite young and have had quite a few x-rays/ultrasounds. All my blood tests which I had also all came up normal which I have attached an image of.

Another thing that came up in the stool test that I forgot to mention was that the M2 Pyruvate Kinase level was ag 6.2 as you can see in the attached image what exactly does this mean and could it be due to the inflammation in my gut?

In terms of that stool test it is quite recent it was only done a couple of months ago just before I went to the US and the colonoscopy was in October last year and gastroscopy in July last year so I am unsure if it’s been inflamed since last year or only recently.

Yes taking any type of fiber substitute would help you they are the bulking agents and help to bulk up your stools.
Only avoid those things to which you are strongly allergic.
Weak one's can be included in diet.
No echo is a non invasibe test.A baseline test of cardiology would hardly take 10 minutes,there is no pain.It involves use of ultrasound but don't worry itscnot at all harmful to you.
M2 p k is a tumour marker used mainly for problems related to gut but sensitivity in not good.Yes inflammatory changes could also elevate it.

Thanks for the reply doctor what do you mean with the M2 p k about the sensitivity not being great? And also in terms of the Gastroenteritis that I got when traveling to the US is it normal to still be having flare ups from it? I seem to get sharp pains before I need to pass a stool which then go away afterward passing it?

But the diarrhea I had while over there is now gone just the sharp pains in my lower abdominal area/pelvic region before needing to pass a stool.

Hello again,
By sensitivity level i mean that the test is not good enough or is not a gold standard test.Results are many times false positive or negative.

As far as gastroenteritis is concerned and as it is recurrent in your case therapy with Rifaximin 400 mg (antibacterial) and Metronidazole 400 mg thrice daily(anti protozoal) should be taken for 5 days.This would eradicate any bacteria or protozoa causing recurrent symptoms.

Start taking bulk forming laxatives like psyllium husk.It absorbs water in intestine swells up and make passage of stool smooth.It is a natural agent so there are no side effects.

Sharp pain in abdomen is due to inflammation of intestines.With every persitalytic moment as intestine contracts and relaxes the inflammed area cause pain.So take a anti inflammatory with antispasmodic for 3 days like mefanamic acid 250 mg in combination with dicyclomine 10 mg twice.

Thanks for that doctor your very insightful I just had one other question but I’ve had this problem for a while now at lest 3-4 years and it may be related but I am finding it’s annoying me more as of recently. Basically everytime I have short bursts of exercise or just walking at a fast pace and then I go talk to someone I find that I am out of breath and have to pause momentarily while I’m talking to take a breath in and then continue talking. I’ve also found that people who wear perfume make me feel like I’m suffocating the same goes for if I wear aftershave I just find it hard to breath and the same for when I’m in places like a nightclub with heaps of people. As I mentioned I’ve also have undiagnosed or “functional” chronic epigastric pain and digestive problems. Now in terms of fitness I don’t really exercise at all which could be a factor but I’m no where near underweight I’m quite the opposite at 56kg and 175cm tall and im only 20 years old. Could it be possible I am maybe having some sort of heart problem that maybe the reason for my epigastric/digestive pain as well or could it astma? I’ve never done any tests for my heart or lungs though other than the doctor checking win the stethoscope when I breath in. I also do suffer with anxiety and stress if thats a factor and I always have this feeling like I’m running on adrenaline could that may be why my digestive system is having all of these troubles especially with my stomach can they all be interconnected?

Yes, it could either be respiratory or cardiac in origin.
Would suggest you two tests-
2d echo and pulmonary function tests. These tests would rule out the involvement of heart and lungs.
Anxiety alone could be a cause of almost every problem.

Hey doctor,

I just wanted to provide an update and a second opinion. I recently saw my gastroenterologist and she believes that I have bile reflux because of the different acid reflux taste and the burning pain I am getting from it. She’s prescribed me Nizatadine 150 to take twice a day that is meant to empty my stomach quicker as I feel like food just sits there and I then get nausea and the reflux that burns. What do you think of this medication and what sort of side effects do they have? Is there anything else I can do for bile reflux?

Hi,
Nizatidine is a histamine H₂ receptor blockers less efferctive then proton pump inhibitors however side effects like withdrawal symptoms are less as compared to proton pump inhibitor.
Yes you can take prokinetic like levosulpiride or domperidon to control reflux.But you would need a prescription for the same.

Thanks for that doctor I’ve also been trying Buscopan which seems to be helping. I have been taking it if I get really bad pain when needed would that be the correct way to take it I don’t feel like I need it all the time? I have just been having 2x Buscopan 10mg when I am in pain is there any side effects that I should I know about this and are they bad to be taken on and off long term will they do any damage?
