Question: I have a hiatus hernia and was on omprezole 60 mg a day but this has not helped as my acid reflux is playing up at the moment so my gp changed me to esomeprazole 40 mg twice a day, how long should this take to work? I have been taking it 3 days now but no change yet

Brief Answer:
Domperidone and other factors as discussed.

Detailed Answer:
Hi.
Thanks for your query.
Noted the history and understood the concerns.
Esomeprazole only reduces the gastric acidity.
Reflux means the stomach contents are coming up in esophagus or even throat.
The are many reasons and associated factors adding to reflux as follows:
Lax lower esophageal sphincter. The laxity allows the stomach contents to come up.
Obesity.
Stress and anxiety states.
Intolerance to certain foods and beverages even some stimuli like the things that are seen or smelt causing one to have nausea or vomit.
* Since you are are already took Omeprazole and now esomeprazole the acidity must have reduced.
* You may please request your Doctor for motility regulator like Domperidone or Metoclopramide (prescription medicines) for control of reflux.
* Alleviate stress and anxiety.
* Identify the foods, beverages, factors that increase your symptoms.
* Small frequent feeds help.
* Reclining position in bed while resting by elevating the head-end of bed .
* Continue the medicines and advise as already given.

All these will help.
If these things fail to alleviate your symptoms then take a second opinion of a Surgical Gastroenterologist.

I hope this answers your query and helps you too.

Take care.
