Question : I just have a new baby 10 days ago and my blood pressure is high my doctor advice to take Ramipril 5mg daily is it safe during breast feeding I already start taking since 4 days and blood pressure is almost normal now,my doctor advice to continue for 1 month

When thinking of drugs prescription in general during breastfeeding, it is important to yield a beneficial effect to the mother, but without potentially harming the new baby.

Such purpose is achieved when using safe therapies, which have shown to not adversely interact with the infant body.

The safest drugs are those which have lowest lipid solubility, highest protein-binding ability, and the shortest half-life.

Now, coming to your prescribed antihypertensive therapy; Ramipril, I would confirm that generally it is not recommended during breastfeeding; as it doesn't fulfill the above mentioned criteria; it may pass into the feeding milk and be harmful.

I recommend you stopping Ramipril from therapy, and discuss with your attending physician for a safer alternative.

Other ACEIs have shown to be safe, like Captopril, Enalapril.

Also Labetalol, Metoprolol, Nifedipine, Atenolol, have no known adverse effects on babies receiving breast milk.

You should consult with your doctor for choosing an appropriate alternative (from the above recommended medications) to be used temporarily until you BP returns to the normal values.

Thank you doctor for your reply,
My gynecologist doctor descriped labetalol,but my cardiologist Doctor descripe the ramipril despite he know I am breast feeding this why I am confused, so can I stop the ramipril immediately and start taking the labetalol.

Ramipril is an attractive and effective antihypertensive agent, but it isn't safe for the baby when on breastfeeding.

I recommend you stopping Ramipril and start an acceptable alternative (Labetalol or others), but first you need to consult with your doctor, and let him know that you want an alternative antihypertensive therapy.
