Question : I have had hemorrhoids for many years. Recently I had a terrible ripping feeling down there when I had a larger poop, and bright red blood. I have had it before. It gets bad for a few days and then stops for awhile. It seems to come back after I rub myself too raw wiping. I checked the area out, and I seem to have a rough are just on the inside of my anus. It does not feel like a pea or a mass or anything. It feels like a scab. It feels like it is on the surface of the skin. Is this normal to get a scab or a rough spot. I am assuming it is a tear that scabbed. I just want to make sure.

Hello,
I am a Gastroenterologist and would be addressing your concerns.
As per your queries answers are-
It can be a fissure which means a breech in the normal mucosal lining of anal or rectal canal.Mucosa means layer of soft tissue which lines anus and rectum.
Second possibility is that it can be an internal hemorrhoid.They cannot be palpated by hands.
So initially i would recommend you to meet a MD and get yourself examined by a proctoscope.Doctor is going to examine your anal and ractal canal through it.
As you feel like ripping it can most likely be a fissure.A fissure is nothing but in simple language means breech in soft internal skin due to excess straining.As it heals up it may form a scab but if this thing repeats again and again it may lead to even condition like dysplasia(initial changes of cancer).
Till the time you see the doctor i would suggest you to take a stool softner daily. You should avoid straining while defecating.Add isaphghul husk fibers 10 mg to your diet.They are bulk forming natural agent and help in smooth passage of feces.Drink 2 glass of XXXXXXX warm water with lemon 10 minutes before going to washroom.All these things would provide favourable condition for the mucosa to heal up.
