Odd array of symptoms over the last two weeks.
Sinusitis - congestion/pressure in nasal passages. Fairly productive as to phlegm.
Dull headache, most profound when waking. Intermittent.
Cold and tingly hands and feet
Sudden onset of achy knees - both of them
Fatigue. Comes and goes, but have taken "cat-naps" most afternoons for last week. Energy level seems to go from normal down to about 75%, then will come back.
Doctor prescribed 14 day course of Amoxicllin-CLAV 875-125, which I am nearly done with. Knee pain came on a week or so ago and is intermittent.
53 year old, physically fit and active man with pretty normal bloodwork from regular annual physical 3 weeks ago. Had blood work done again last week and results came back with some changes, namely LOW WBC (3.9 on a 4.2-11.8 reference guideline. Previous result on 6/15/18 was 4.7), MONOCYTE % was HIGH (11.3 on a 3.8-10 reference guideline, was 11.0 on 6/15/18). MONOCYTE # on 6/15/18 was 0.5, now it is 0.4. Protein Total is LOW at 6.4, which is where it was on 6/15/18.
Need through check up followed by specific medication.

Hi triguycave,
Thanks for the query. I have gone through it & understand your concern.
Overall impression after reading your complaints is - you have allergy super seeded with a viral infection .. all of your symptoms can be explained by the assumed diagnosis.
- Viral infection had attacked already weakened sinuses( Infection is mostly self limited within a weeks time & when it does not, it needs antiviral medication ) --- to cause sinus problem with dull head ache with waking ( follows sleep disturbances due to sinus infection/ blocked nose/ snoring )
-Tingly hands & feet , pain in knee joints & fatigue follow viral infection.
- Low WBC count results from bone marrow depression by virus.
- Low blood protein could be due to less intake/ less digestion ( intestinal problem )/ less production( liver problem )or more loss ( viral fever affecting kidney )
* You had already taken 2 weeks course of antibiotic Amoxillin- Clav without any improvement , The total WBC count is low, monocytes have increased to compensate the defense but probably not useful because of resistant strain of bacteria / viruses not being opposed by antivirals.
* In this scenario
- Blood investigations for confirming viral infection followed by suitable antiviral. antiviral medicine as per need
- Improving protein & blood problems by blood transfusion & high protein diet as per need.
- Culture& sensitivity test of nasal passage discharge to find any secondary bacterial infection followed by suitable antibiotic as per need.
- Addition of anti allergic medicine to compliment the treatment with antivirals/ antibiotic.
- Improving nutritious diet along with vitamin supplements.
-Self care with avoiding allergens/ taking rest/ vapor inhalation
would help.
* The management depends on your own health need so there is need advise & care of an ENT specialist/ physician for methodical management of problems.
I hope that helps you. Ant follow up query is welcome.
Thanks.

Detailed Answer:
Hi Triguykev,
Thanks for the follow up response.
There are enormous strains of viruses which can cause multi organ involvement& cause the symptoms... some of them are Influenza virus,Echovirus .
The type of affecting virus can be detected by physical examination co related to current predominant infection in area/ blood test & suitable antivirus medicine can be chosen.
I hope that satisfies you. Any further query you have , is welcome.
Thanks.
