Hi,
Split tongue is a fissured tongue. Mostly a long fissure is seen along length of tongue to make it have a split look. It can be accompanied by few smaller fissures. This can be from birth (dawns syndrome) / due to malnutrition or infection / inflammation of tongue (bad oral hygiene).
In your case as it is painful and reaches up to ear. It seems to be bad oral hygiene and infection where both tongue &eustachian tube (joining oral cavity with middle ear and can carry infection there to cause pain in ear.)are affected.
* Management consists of self help by improving oral hygiene -
- Having regular cleaning of oral cavity. Brushing of teeth regularly, gargling after every small serving of food, avoid sticky food. Thus avoiding &getting all the dental problems treated.
- Cleaning of tongue with a tongue cleaner / own fingers in your present condition when tongue is already inflamed or infected with a gentle massage &gargles with warm water.
- Massage of gums after each tooth brush would help in keeping gums healthy
- Avoiding/managing infection in oral cavity by prevention or treatment of sinusitis, tonsillitis, inflammation of adenoids.
- Avoid/stop smoking or chewing tobacco.
- Having good bowel clearance (bad digestion, infection of bowels or constipation can destroy normal intestinal flora, lactobacillus and cause vitamin B complex deficiency and tongue ulcers, these get infected and swollen due to bad oral hygiene)
*In this scenario along with improving oral hygiene you would need to remove basic infection and infection of tongue by medication and control of all above factors.
