Question : I just had a fairly large and hard bowel movement that was a lot of "hard balls" packed together. It was hard to push out and painfull. It had some blood on one side of the stool, and now my rectum is like burning and feels like pins sticking me. I just had a second bowel movment that was softer and a fair amount that did not have any blood. Could I please get your thoughts.

Have gone through your details and i appreciate your concerns,
When ever hard stool passes through rectum it may breech the thin layer of soft tissue lining it called mucosa.So mild bleeding could be there.Hard balls packed together like appearance is clearly suggestive of lack of fibers in diet.You should increase your fiber intake to a minimum of 30 gram per day.You could add isaphghul husk 10 gram twice daily in your diet.Its a natural husk and imbibes water swells up and help forming bulky and soft stool which passes easily through intestine.Soft stools followed by constipated hard moments is a natural way by which intestines try to help relieve constipation or further impaction.Intestines secrete large amount of mucous to lubricate the intestinal tract hence helping stools to pass and prevent impaction.
Pain stinging and burning sensation in rectum is due to the breeched mucosal layer.I would suggest you to take a laxative like Miralax in low dose 10 ml per day to keep your stools soft(Over the counter).This would give time to your mucosa to heal.
Add isapghul husk to your diet.
Take probiotic daily.
Take lots of butter milk or yakult in diet(active lactobacillus cultures).
Drink 2 glass XXXXXXX warm water with a spoon of apple cider vinegar in morning.Water will help in smooth passage of stools.
Don't strain a lot while defecating as this may result in rebleed.You can take laxative upto a dose of 30 ml safely.
Eat a bowl of three seasonal fruits(orange,pear and apple) with fiber rich vegetables daily.
If buring persists an anti inflammatory like Ibuprofen can be taken.

Thank you for the information. I just went again and only noticed a small amount of blood on tissue. And really burning I forgot
To add Thai had been digonsed with IBS years ago and had a anal fisher back then.

Hello again my dear patient,
As you are a diagnosed case of anal fissure you should try to keep your bowel moments as smooth as possible.Whenever the stools are hard and you need to strain;The fissures would start bleeding.Fissure is nothing but a medical term given to recurrent breech of mucosa.
