Question : Hi Doctor...I've been experiencing nausea, fatigue for the past 3 weeks. Initially also had an intermittent warm sensation in the lower left abdomen/upper groin.
Had blood work done and showed elevated liver enzymes...specifically ALT was 122, then 92 2 days after that...then down to 66 and then back to 72
CT scan showed no diverticulitis or anything else remarkable. Microscopic hematuria in urine. Negative for Hep A.
Also been experiencing night sweats, and foul gas the last 2 days.
My care team hasn't been able to discover anything...any thoughts appreciated

Hello,

Have seen your details and I appreciate your concerns,
Foul smell in stools is suggestive of incomplete fermentation of food. Whenever food stagnates in intestines it is acted upon by a group of bacteria fungus and other organisms which start to form their own colonies there. You have a history of GERD and gastroparesis. This makes you even more susceptible to such colonisations and infections.

Was a stool routine and microscopy and culture sensitivity test done on you?

Another possibility can be SIBO. Small intestinal bacterial overgrowth (SIBO) is defined as the presence of excessive bacteria in the small intestine. A stool test should, therefore, be done.

I would suggest you take-
- Probiotic VSL#3.
- Take a course of broad-spectrum antibiotic like Rifaxamin (you would need a prescription for that).

Less than 3 times normal elevation of liver enzymes has no clinical significance.

Take a laxative daily for a few days to clean up the gut.
