Hello,
Welcome to 'Ask a Doctor' service!
I understand your concern and would explain that these ECG findings could be related to a lung disorder or a cardiac disease. I would like to know more about your complaints:
- Have you experienced shortness of breath?
- What is your past medical history?
- Have you had any lung disorders in the past?
Coming to this point, I would recommend performing a cardiac ultrasound, a chest X-ray study and pulmonary function tests. You should discuss with your doctor on the above tests!
I remain at your disposal for any further questions whenever you need!

Detailed Answer:
Hello again,
The shortness of breath after smoking could be indicative of a lung disorder. For this reason, I would recommend performing a chest X ray study and pulmonary function tests. A cardiac ultrasound is also important to examine your heart function and structure.
Regarding RDW SD, I don't think that it is related to these ECG findings. It is indicative of anemia. For this reason, I recommend checking iron plasma levels.
Hope you will find this answer helpful! Wishing you all the best.
