How To Resolve The Foreskin Shrinking At The Top Problem ?

my age is 30 and i am married. I have some problem in my foreskin it's shrink at he top, the mouth is not opening if i tried there will be cut on it. Even i am unable to do sex,Please tell what kind of problem is ithis and how can i resolved it.(any medicine or tube),
