How To Treat Irritated Area In The Intergluteal Cleft?

How would you treat and irritated area in the uppeer intergluteal cleft. It started from laying in bed and now the biggest issues is moisture. We started with barrier cream, the MD ordered betadine BID and LOTA (not a fan of this treatment) and now their are 2 dine sized open areas on the r side. Would you try an alginate?
