Solution For White Patches In The Chest?

From last 2 years I am having many areas of hypopigmentation over my chest and back. When d first time I took it got cured but after I stopped t/t these white patches reappeared since then I have taken t/t twice but its not getting cured.....my KOH test was positive nd doctors say it is pityriaris versicolor...plz tell me a good t/t???
