Suggest Remedy For Anal Pain

I am 63 years old. Anal pain for one year. Had sphinterotomy (LIS) in Sept last year followed by ligation of internal hemorrhoids in December. Intense pain spasms followed but had subsided in January. This was replaced by awful soreness, itch. Been taking fibre everyday and sometimes Dulcolace to soften stools. My doctors in Singapore (both hospital professors) in don't seem to be able to help. Taking gabapentin and hydroxyzine for relief but still tortured by sore/itch/pain everday after defeacation. What can be wrong with me? Can you recommend a good proctologist in Singapore who may be able to help.?My life is hell thinking of suicide . Seen by psychiatrist and pain speciatist. but of no help.
