Suggest Treatment For A Red Spot On The Shoulder Region

My son awoke this morning with a red quarter-sized spot on his left shoulder blade; he apparently scatched it leaving the center (1/16th") raw. He does not have a temperature nor discomfort (other than the very raw center).

What could it be and how should it be addressed?
