What Causes A White Tissue Like Discharge During Urination?

Hi Dr I m Ntokozo. I have a problem and I think this is my 4th/5th year its been going on....I have a discharge and sometimes its smells very bad i ve had antibiotics for several times but not change. Well today in the morning I woke up and went to the bathroom to pee, strange thing I noticed some white tissues in my urine and I m confused I don t knw what s happening my last periods started on April 20 til the 26th (I have irregular periods by nature) and I had sex on the following days:30th-04;12-05&14-05 so I haven t received my periods for this month(May). Dr what could be wrong with me...are there any possibilities I could still be fertile after all? Or I might be having another disease help please?
