What Causes Continuous Vaginal Bleeding?

i have a heavy bleeding for over a month, sometimes its very bad and sometimes its ok,im scared to even leave my house,as my doctor told me im also anaemic whats the cause of me bleeding so much,i also pass big clogs of blood...is it possible i miscarriaged???? what should i do?
