What Causes Enlarged Lymph Nodes In The Chest And Groin?

My father was diagnosed with hurthle cell carcinoma 5 cm tumor. Had a trach put in and he is 78 yrs. old. They have found enlarged lymph nodes in his chest and one in his groin area. Does this mean that the cancer has spread? And if so how long does he have to live?
