What Causes Heavy Bleeding And Cramping During Periods And Can Any Exercise Will Help?

I am 37 years old, started my period when I was 8. I am 5’10”, 250 lbs. When I was in my teens- mid-20’s, I was very overweight and only had 3-4 periods a year at irregular intervals. I have since lost a good deal of weight (125 lbs) and my cycle has been regular for years (31-32 days). I am a little concerned because it seems like things have been changing from what was my normal cycle in the past year or 2. I have always had cramps, but in the past couple of years they are much worse. The cramps start off and on about a week to 10 days before my period starts (it used to be maybe the day before, sometimes none until my period started). I get nauseated, and the cramp pain goes down my thighs. The cramps last until about the 4th day of my period now; before it was just the 1st day. My flow has gotten heavier and it has lots of clumps, and my period now lasts 6-7 days instead of 4-5. I also have new cramping pain during the middle of my cycle, I guess when I ovulate. I also have severe cramping pain sometimes when I orgasm, but not always. It feels the same as cramps, but much worse pain.

I am going to the gyn next week for the first time in about 6 years. What should I ask? What would be helpful for her to know?
