What Causes Pain In The Ear And Tightness In The Shoulder?

I have been having odd stroke like symptoms on and off for a year along with pain in ear, tightness in neck shoulder and sever headaches. This often comes with severe weakness and flu like feelings occassional chest pains.. MRI of brain is normal. ears check out normal, I was told it was not a stroke and so far I am fine. For the past week all those symptoms are back with a lump feeling in my throat. It feels as if I have colliflower stuck on the bottom of my tonsil when I swallow. It is rather annoying and I feel like I have the flu. I am very short of breath.
