What Could Penile Skin Burning And Cracking During Erection Indicate?

I am 52 and taking blood pressure tablets. I have been having a problem with my penis skin cracking and burning durning erection also goes red and which is very painful and also the skin around the upper part of the shaft under the head feels very tight. This has been going on for about 3-4 months.
Please can you help me as it is making me avoid having sex.
I use cream after and it does not hurt when flaccid only when i get an erection.
