1. Moderate coronary artery calcification, with the patient nearly at 70th percentile adjusted for age. 2. Accessory small LAD coronary artery arising from the right coronary cusp, with an inter-arterial course and appearing patent. 3. Mixed plaque formation in the mid left circumflex coronary artery, producing mild to moderate-grade stenosis. Mild to moderate-grade stenosis in proximal OM1, produced by calcified plaque. No significant stenosis within the right coronary artery or the LAD. Comment: An inter-arterial course of the LAD can be associated with ischaemia and cardiac arrhythmias and sudden cardiac death and is classified as a malignant coronary anomaly. There is a true LAD which supplies the major distribution of the usual LAD territory, however the LV apex appears to be supplied solely by the accessory LAD.
