18 Month Old Started Moaning In Sleep In Middle Of

18 month old started moaning in sleep in middle of night but didn't wake up until we woke him. His skin is red and hot. 104 fever with forehead thermometer. Gave tylenol but it helping yet. He' breathing heavy and a little forceful. No coughing or runny nose. Usually his eating declines prior to getting sick, but he's been fine. He was diagnosed with Pfapa several months ago but hasn't been getting sick nearly as often anymore. He's just weak and laying there trying to sleep but can't.
