3 Year Old Boy, Complains Not Feeling Well Specifically His

3 year old boy, complains not feeling well specifically his belly. Kept saying he had to "poop" and then didn't. Incontinent loose stool four times finally got on toilet and had explosion. Not really formed. That was two days ago. Threw up 3 times last night. Poor appetite. Drinking lots of water. Threw up after milk. No fever. No abdominal swelling. Lack of energy. I'm a nurse and I'm afraid he's impacted. Trying to give him juice, not very successful. Enema is out of the question. Thinking about a stool softener. What are your thoughts?
