3years Ago I Had A Mesh Removal, Removed A Sling

3years ago I had a mesh removal, removed a sling a replaced with a new sling using my own facia. I had some muscles cut in the vaginal wall. Now, when ever my boyfriend and I tried to have sex it hurts so bad. We can't do it because even if I try to go slow it feels like he's cutting me so we stop and I notice blood all the time. We never get half way in either it's so painful. when i had a doctors appointment to look at my kidneys i had to have a catheter. They could not get the catheter in for almost an hour. One nurse suggested to try a speculum and another nurse said i was so tight there that she couldn't get her pinky in and wouldn't do any good. Is this from my previous surgery? And can it be fixed? I also have a tilted uterus.
