46 Year Old, Male With High BP (controlled With Daily

46 year old, male with high BP (controlled with daily meds), low testosterone (controlled with weekly injections), IBS, acid reflux are most of my health issues. I have had a type II heart attack during a surgery to repair a tear in my shoulder. Since then, I have had my WPW corrected via an ablation. My father had several strokes, then died of cancer (13 years later) and mother takes meds for an irregular heart beat, diabetes and high bp... I just noticed that my nipples look gray and both have red rings around them. Is this something I should have looked at or could it somehow be due to my physical activity increasing after a year of hardly any. I retired from the Marine Corps last year and went from running 5 miles, 3Xs a week to no running at all. I used to regularly lift weights and stopped that also. I am 6' tall and currently weigh 295 lbs. (from the 225 I was last year).
