63-y.o Male Diabetic Insulin Dependent Stroke Victim Who Had A

63-y.o male diabetic insulin dependent stroke victim who had a massive stoke March of 2014. Affected speech and after 9 months of 3x a week therapy, can talk but has a lot of aphasia and days which are better but not much. Had a TIA yesterd with blood pressure of 86/54 and up to 140/70. Went to ER and did a CT scan but not anything new showed up. Given Plavix and saline. Better today but understandably tired. Other indications were loss of sight temporarily as well as lack of easy movement in the right arm. Would like to know if there is anything we should watch for in particular that might help including stem cell treatment. Will he have another TIA. Does DM make it worse?
