A Patient That Has An Iv Shunt Placed From The

A patient that has an iv shunt placed from the brain to his abdomine over twenty two years -now within the two years have bowel obstruction from scar tissue of the shunt - 2 times lucky not to do any surgery but was able to have a tube placed from the nose to the abdomen to clear up
Do we try to remove old shunt and get direction from first neurosurgeon who did the shunt first and two weeks removed brain tumor of 1994?
What if any can we avoid future emergency - not sure if we will be lucky third time
Can we do periodic scans to avoid as there seems to be no prevention of this happening again

Please advise
