Are Raised, Itchy, Red Bumps On Top Of Hands Of A Teenager A Cause For Concern?

My 14 year old son has raised itchy red bumps on the top of his hand. It seems to be spreading as he forgets to use the Cortizone cream I asked him to use regularly. I m assuming it is some form of eczema but not sure if there is anything additional we should be doing to help clear this up. The bumps are small and started on the top of his finger and extended to the top of his hand.
