Are Topical Creams Sufficient To Treat Fissures?

I am currently being treated for fissures and am applying topical creams. There is also a red rash between the buttcocks that burns. Because there is discomfort in this area, sitting is uncomfortable and may be contributing to soreness in the perineum (?) region. Does this make sense?
