Back In October I Was Tested Positive For Chlamydia. I

Back in October i was tested positive for chlamydia. I however was active too soon after treatment and since then have taken azithromycin two pills 500 mg 5 times. This month January being the 5th i was retested but have yet for my results i went to a free std clinic i thought i had an uti as i took at home uti test which came back positive. The doctor said had something called cervicitis thats why i was given the pills this time. I was wondering if i took azithromycin too many times and it will not work. Also is it possible that i have gotten chlaymdia again my last test result i had last month was negative. I am worried if i will not be able to have children and scared the chlamydia will not go way. does cervicitis mean my Fallopian tubes and uterus are damaged now ?
