Boyfriend Has A History Of Chronic Debilitating Migraines, And Recently

Boyfriend has a history of chronic debilitating migraines, and recently had surgery for nasal polyps. was recently on antibiotics for the later, still occasionally taking pain meds for the former. Has been badly constipated today since about 2 pm (time now almost midnight). Tried an enema with only partial success. Vomited earlier when nothing was passing. Now has a slight fever of 100.2. Not describing any stomach/abdominal pain per se, but now is also getting a bit of a headache (though not saying it's migraine level yet). At what point should he be heading to the doctor or emergency room?
