Can Insomnia Cause Bowel Incontinence And Dizziness?

I have insomnia. I have not slept more than 3-6 hours in past 3 months, but sometimes days and days without sleep. I am not bi-polar. The doctors have given up on treating me. I fear safety for my own health. When does this problem neccessitate mental hospitilization as I have once but was never treated for shocklike symptoms (loss of bladder bowel control, sweating, headspinning) from not sleeping?
