Can An Ankle Patch With Scarring, Itching, Flaking And Bleeding Be Treated?

I have a patch on the outside of my ankle that flares up - there is scarring but it itches so badly that I scratch it til it bleeds. I also have skin that flakes off with it. What is it, what can I do - I have tried anti-itch Eucerin - which works for awhile.
