Can You Tell Me What's Wrong? My Nephew Has Been

Can you tell me what's wrong? My nephew has been feeling sad for over a year. He was diagnosed with major depression and physotic episodes. He's takes medication. But I thought he would be back to normal by now. He keeps to himself, moves real slow, has a problem remembering and nothing seems to excite him.
