Concerned About Heart Problems. My Friend Has Been Sick For

Concerned about heart problems. My friend has been sick for 7 days. Possible recurrent fevers but average temperature today. Fatigue, chestpain, shallow short breathing, clammy and coughing. Pulse normal. Fluctuating blood pressure. Highest measured in past few hours is 164/98 and lowest 89/47 in the last few minutes. Typically diastolic is main concern. What can I do to help? When is it time to go to the Emergency Room?
