Could Atrophied And Blackened Toe Be Due To Fracture?

have had beginnings of PVD/diabetes resolved with gastric bypass post 2 years. in past lost left toenail with crush injury. Have just last week had right great and second toe rolled over by wc. Second toe top beyond DIP blackened but also seems cold and atrophied, joint blanched. Should I be more concerned than to believe that it is broken? Have high tolerance for pain but also routinely take med for pain/inflammation due to fragment that flares up sciatic nerve on Left. Last year's MRI showed several discs prolapsed but they should have been resolved by now.
