Could Hiding My Smoking History From The Anesthesiologist Before My Nose Surgery Cause Serious Complications?

Been smoking from last 14 years. 28 yrs old right now. Going to have a nose job in some days most probably under general anaesthesia. Stopped smoking 3 days ago and surgery in two days. I am not sure that if i can share my smoking habit with my anesthesiologist. If i hide this will there be any fatal complications?. I cant share because chances are that my family might know.
