Could Severe Pain In Neck And Chest Be Due To Pulmonary Embolism?

I have a DVT in my left leg and I am on warafin and have been on it since the end of January and been having trouble keeping my INR at a therapeutic level and now I m having severe pain in the back of my right shoulder my neck and chest pain on my right side could I have a pulmonary embolism while being on warafin ? If so should I go to the emergency room since I m already on warafin?
