Could Symptoms Like Testicle Shrinkage, Hardening Of The Vesicular Tube, Swelling, Pain, And Changes In Semen Indicate A Serious Issue?

Hi Doctor. My right testicale has shrunk, on the right side the visicular tube is very hard. Some times the right side swells up and is tender and becomes painful, lately it can get quite severe. Sometimes it is painful when urinating. The lower abdomen can have mild to severe pain. It can have a sense of heaviness and discomfort. Also when have sex can be painful and when ejaculating it s painful. My semen has become watery and not full and lots like before. Also my penis seems to have retracted and does not get as hard as use to. I also feel my pelvis is uneven. I am 49 yrs of age, a bit of pot belly but nothing over weight. Haven t been excesising in the last 2-3 years. I use methamphanine and sex tablets. I did had ultrasound 7 months ago it showed the visecular tube thicken but couldn t determine anything else. I was referred to have biopsy but haven t done one yet. This started may be 9 months ago first the tube being hard then testical shrinking then the swelling and pain. Can you advise please
