Could Urinary Incontinence Be Due Slipped Sling?

I had a vaginal hysterectomy and a suspension of my bladder 5 or 6 years ago. Now I have incontinence which I cannot control. I had a sling or other device put in. Could this device have slipped out of the correct position and be causing problems? And also how if this is what happened is it diagnosed?
