Dear Doctor, My Wife Has Been Suffering From Migraine For

Dear Doctor,
My wife has been suffering from migraine for last 10 years. She's on medication (Electriptan- 40mg, Topiramate-50 mg, Lexapro-10 mg; 1/2). Her recent MRI report has the following:
"Prominent cerebral cortical sulci otherwise MRI of brain revealed no significant abnormality"
Despite all the above medication, she has periodical (2-3 times in each month which last for 2-3 days) severe headache. Kindly Advise.
