Dear Dr I Have Been Having Itching And Dark Spots

I have been having itching and dark spots my feet and skin near ankle since a year now...There are cluster of hard & roughskin that cause lot of itching. The same spot i have noticed on my finger. After consulation from many dermatologist over last many months and they advising me steroid based ointments (Eg Myconid , Clolite-ZE, Prophyslic and vozet) the relief has been temporary and not permanent. I have also been applying A-Derma mosturiser on and off..I do wear shoes for 10-12 hours a day and cant avoid normal sweat.
One of the doctors also suggested me to go for skin biopsy to determine exact cause of problem which i avoided and have not got the same done till now.
Pls suggest what treatment i should follow. Should i go for skin biopsy or not? Pls confirm.
