Does Semen Leakage During Urination And Scrotum Pain Affect Sexual Activity?

One of my friend Gopalkrishnan is suffering from semen (transparent liquid) leakage with urine and sometimes white sperm. Left side testes size bigger than right testes. Sometimes his low abdomen and scrotum feels too much pains, but when he masturbate an ejaculates the sperm the pain will be OK. Do this affect his sexual activity and health? Because he is also suffering from premature ejaculation. Thanks in advance for your kind answer and advise.
