Dr. Chandan, Last Year While Having Sex With My Wife,

Dr. Chandan, last year while having sex with my wife, some scratches occurred on my penis. It was on the skin. Again this year there were cracks and finally developed small sour near by my penis head. It is painful when it comes in touch with my sperm while having sex. Please advise me. I don't think i can visit doctor.
