Feb 2014 My GFR Was 53; Aug 2015 It Was

Feb 2014 my GFR was 53; Aug 2015 it was 49; latest lab in May 2016 my GFR is 34. My Creatinine is 1.98. My primary care physician is well aware of my history over the past 30 years. I am a 68 y/o male; history of 5 back surgeries and 4 foot surgeries over the past 15 years. My BP is well under control. I'm hesitant to see a Nephrologist. Am I being too cautious? Is it time to accept the referral?
