Female Age 76 Have Many Conditions My Internal Med Physician

Female age 76 have many conditions my internal med physician is treating me for, however she is currently on
Maternity leave and I need to determine what kind of a specialist to contact for these new conditions now.
Approximately two to three months ago I noticed my middle finger developed a knot on the joint just below the nail bed, about the size of a pea. Kind of touchy but not painful. That finger joint bent similar to hands with RA.
Four weeks ago the entire finger became extremely painful followed by the ring finger becoming just as painful
with pain and cramps running up and down the shoulder and arm. The other hand began to mimic what was
happening in my left hand and arm, but without quite as much severity. The pain was so intense I began to
equate it with some of my bouts of gout and really thought that was what was wrong. My fingernails began to
feel a painful pressure as well as the finger tips felt as tho blood was rushing to the tips every time I moved my
fingers or hand and was stinging and burning. In the last few days the palm of my hand has become so tender
I cannot use it to assist getting up and down (I am in a wheelchair unable to walk further than three to four steps).
I've reached the point of desperation, my nerves are overwhelmed and I cannot think of anything but how
uncomfortable and painful all of this is. I should mention that I have a ruptured disc, as I recall it is the first disc
in my spinal column. Also, certain movement of my arm, shoulder and neck causes shooting pain in that area.
