For 5 Months Now, I Have Been Getting Small Chest

For 5 months now, I have been getting small chest pains and faint spells that come and go all day everyday. I constantly belch loud and it's hard to breathe. It affects everything in my life and I been to the hospital 4 times, and they said it could be acid reflux. But none of the medication had helped. I'm scared and I need answers now!
